Website notice
Privacy Policy
What this website collects, what it does not, and what happens to anything you send through it.
Effective date: October 22, 2026. This policy describes the website of Hawthorne Mental Health LLC. Health information held by the practice is covered separately by the Notice of Privacy Practices.
1. Who this applies to
This policy applies to Hawthorne Mental Health LLC, trading as Hawthorne Psychiatry, and to the website at hawthornepsychiatry.com. It covers the website only.
Protected health information created or held in the course of providing care is governed by the Health Insurance Portability and Accountability Act and is described in the Notice of Privacy Practices, not here.
2. What we collect
This is a small set of static pages. There is no account to create, nothing to log in to, and no database of visitors.
- Nothing, by default. Simply reading these pages does not create a record that identifies you.
- What you choose to send. If you use the contact form, your message is emailed to the practice inbox and a short automatic reply is sent to the address you give. The website does not keep a copy of the message or a database of enquiries.
- Ordinary server logs. The hosting provider records standard request information, such as IP address, browser type and the page requested, for security and reliability. These are the provider's logs, kept under their retention policy.
Please do not send symptoms, diagnoses, medication details or other sensitive information through the form or by email. See section 6.
3. How it is used
Anything you send is used to answer you and to arrange care if that is what you have asked about. It is not used for marketing, it is not sold, and it is not used to build a profile of you.
5. Cookies and tracking
This website sets no advertising cookies, runs no analytics that identify you, and carries no social media or advertising pixels.
Google Maps on the contact page. The contact page shows an embedded Google Map of the practice's location. When that page loads, your browser connects to Google to draw the map. Google receives your IP address and browser details, may set its own cookies, and handles that information under Google's privacy policy, not this one. No other page loads anything from Google.
If you would rather not load the map, the address is written out in full on the contact page and in the footer of every page, with links that open your own map app only when you choose to. No video or social feed is embedded on any page.
6. Email and web forms are not secure
Ordinary email travels through systems neither you nor the practice controls, and can be intercepted, misdirected or read on a shared device. The same is true of a message typed into a web form.
Please use email and the contact form only to ask about making contact. Keep clinical detail, history and anything else sensitive for your appointment, where it is properly protected. Never use either in an emergency: call or text 988, or call 911.
7. How long it is kept
Enquiry emails are kept for as long as needed to deal with the enquiry and to meet the practice's record keeping obligations, and are then deleted. If your enquiry leads to care, information forms part of the clinical record and is retained under the rules that apply to health records.
8. Your choices
- You can ask what enquiry correspondence is held about you, and ask for it to be corrected or deleted where no legal duty requires it to be kept.
- You can ask to be contacted by a particular method, or at a particular number, or not at a particular number.
- Rights relating to your health record, including access, amendment and an accounting of disclosures, are set out in the Notice of Privacy Practices.
To exercise any of these, use the contact details in section 11.
9. Children
This website is intended for adults. It is not directed at children and does not knowingly collect information from them. If you believe a child has sent information through this site, please contact the practice so it can be removed.
10. Changes to this policy
If this policy changes, the revised version will be posted on this page with a new effective date. Continuing to use the website after that date means the revised policy applies.
